Senior Integrated Community Children's Nurse

The Community Children's Nursing Team at Solent NHS Trust is seeking a highly skilled and experienced Senior Integrated Community Children's Nurse to join our dynamic team. As a key member of our team, you will be responsible for providing high-quality, family-centered care to children and young people with complex health care needs and disabilities.
Main Responsibilities- Assess and develop individualized care plans for children and young people with complex health care needs and disabilities.
- Provide specialized, high-quality nursing care, considering the physical, psychological, spiritual, and social needs of the child.
- Work collaboratively with families, carers, and the wider multi-disciplinary team to ensure care is provided as close to home as possible.
- Lead and develop care plans, training, and multi-disciplinary liaison for children and young people with a range of needs within their home, school, and wider community.
- Manage a caseload and provide supervision and monitoring of junior staff with support of team leader.

Person Specification- BSc Nursing or RN in Child or Learning Disability Nursing and currently registered with NMC CCN Specialist Practitioner Qualification or willingness to complete within 2 years of being appointed.
- Experience in assessing and developing patient care plans, developing and delivering care and clinical interventions to patients, and experience of MDT working.
- Clinically competent at Band 6 level, excellent communication skills, written and verbal, ability to receive and provide complex information which may be sensitive, contentious, where persuasive negotiation may be required and there could be barriers to understanding.
