Senior Integrated Community Children's Nurse

The Integrated Community Children's Nursing (iCCN) team at Solent NHS Trust is seeking a highly skilled and enthusiastic Senior Integrated Community Children's Nurse to join our team. As a key member of our team, you will provide specialist nursing care, support, and coordination of care to children and young people with complex healthcare needs and disabilities.
Main Responsibilities- Provide high-quality, holistic, family-centered care to children and young people with a range of needs, including acute and short-term conditions, long-term conditions, disabilities, and life-limiting illnesses.
- Develop and implement individualized care plans, working collaboratively with children, young people, families, and the wider multidisciplinary team.
- Lead and manage a caseload, providing specialized nursing care and considering the physical, psychological, spiritual, and social needs of each child.
- Support the management of integrated acute and community care pathways for children with short-term conditions.
- Provide supervision and monitoring of junior staff, with support from the team leader.
- Work with families to provide emotional support and specialist clinical care, sensitive to the changing and ongoing needs of each child and their family.

Person Specification- Essential qualifications: BSc Nursing or RN in Child or Learning Disability Nursing, and current registration with the NMC. CCN Specialist Practitioner Qualification or willingness to complete within 2 years of appointment.
- Essential experience: Experienced in assessing and developing patient care plans, developing and delivering care and clinical interventions, MDT working, teaching, training, and supervision/assessing junior staff and students.
- Additional criteria: Clinically competent at Band 6 level, excellent communication skills, written and verbal, ability to work autonomously, prioritize workload, instruct and direct others, work is managed rather than supervised, problem-solving skills, advocacy skills, teaching, supervision, and assessing skills.
