Senior Integrated Community Children's Nurse

We are seeking a highly skilled and experienced Senior Integrated Community Children's Nurse to join our team. As a key member of our integrated Community Children's Nursing (iCCN) team, you will provide specialist nursing care, support, and coordination of care to children and young people with complex healthcare needs and disabilities.
Key Responsibilities- Manage a caseload of children and young people with complex healthcare needs and disabilities, providing high-quality, skilled nursing care that meets their physical, psychological, spiritual, and social needs.
- Develop, implement, and evaluate individualized programs of care, working closely with families, carers, and other healthcare professionals to ensure seamless care coordination.
- Support the management of integrated acute and community care pathways for children with short-term conditions, ensuring timely and effective care transitions.
- Provide supervision and monitoring of junior staff, supporting their professional development and ensuring high-quality care delivery.
- Work collaboratively with families, carers, and other healthcare professionals to provide emotional support, specialist clinical care, and education on complex nursing care and procedures.

Person Specification- BSc Nursing or RN in Child or Learning Disability Nursing, with current registration with the NMC and a CCN Specialist Practitioner Qualification or willingness to complete within 2 years.
- Experience in assessing and developing patient care plans, developing and delivering care and clinical interventions, and working in multidisciplinary teams.
- Excellent communication and interpersonal skills, with the ability to work autonomously, prioritize workload, and instruct and direct others.
- Problem-solving and advocacy skills, with the ability to adapt to change and manage emotional issues.
- UK driving license and use of a car to carry out community visits.
