We are seeking a highly skilled and dedicated Care Coordinator to join our team at Black Country Job Support. As a Care Coordinator for Early Intervention in Psychosis, you will play a vital role in supporting individuals who have experienced psychosis or are at risk of developing it.
Key Responsibilities- Provide specialist assessment, planning, and intervention to individuals with psychosis or at risk of developing it.
- Work closely with multidisciplinary teams to develop and implement individualized care plans.
- Engage with individuals and their families to promote recovery, well-being, and empowerment.
- Contribute to the development and delivery of evidence-based interventions and programs.
- Collaborate with other agencies and services to ensure comprehensive care and support.
- NMC or HCPC registration.
- Postgraduate clinical experience.
- Interest in working with individuals who have experienced psychosis.
- Community mental health experience.
- Experience of working with families.
- Completed training in Behavioural Family Therapy.
