We are seeking a highly skilled and experienced Registered Mental Health Nurse to join our Specialist Dementia and Frailty Team as a Nurse Practitioner. This is a community-based position that requires the ability to drive and have access to a car for work purposes.
Key Responsibilities- Assess, plan, implement, and evaluate packages of care for patients with dementia and frailty, in line with professional codes of conduct and Trust policies.
- Work as a member of the Mid-Dementia and Frailty Pathway, providing high-quality, evidence-based, holistic care to patients.
- Supervise and manage junior team members within the professional group and team locality, as required by the manager.
- Registered Mental Health Nurse (RMN) qualification.
- Experience in performance management and supervision of lower-banded staff.
- Understanding of health legislation, principles of CPA and Care Management, Mental Capacity Act, Mental Health Act 1983, and safeguarding.
- Full UK driving license and access to a car for travel.
