About the Role

The Sussex Community NHS Foundation Trust is seeking an experienced Therapist/Allied Health Professional to join our Admission Prevention Team at the Royal Sussex County Hospital. As a key member of our team, you will play a vital role in preventing hospital admissions by working closely with A&E staff to ensure seamless, safe, and effective discharge plans for patients.

Key Responsibilities

  • Utilize your clinical expertise to support A&E staff in the management of discharging patients into the community and community beds.
  • Support with the induction of new staff and attend MDT meetings as required.
  • Provide training and education to A&E staff on community services and discharge management processes.
  • Maintain accurate records and data in a timely manner, adhering to information governance and data sets required for the service.
  • Offer advice and support to patients and carers on healthcare matters, utilizing your clinical expertise to facilitate rapid discharge and prevent hospital admission.

Person Specification

  • Professional qualification in Physiotherapy or Occupational Therapy.
  • Current registration with the Health and Care Professions Council as a Physiotherapist or Occupational Therapist.
  • Evidence of relevant postgraduate courses/CPD and reflective practice relevant to clinical specialty.
