About the Role
We are seeking an experienced Therapist/Allied Health Professional to join our Admission Prevention Team at the Royal Sussex County Hospital. As a key member of our team, you will play a vital role in preventing hospital admissions by working closely with A&E staff to ensure seamless, safe, and effective discharge plans for patients.
Key Responsibilities
- Utilize your clinical expertise to support A&E staff in managing discharges and preventing hospital admissions.
- Work closely with A&E staff to identify potential improvements or changes within the service to support effective safe discharge.
- Provide training and education to A&E staff on community services and discharge management processes.
- Maintain accurate records and data in line with information governance and data sets required for the service.
- Offer advice and support to patients and carers on healthcare and clinical expertise.
