Job Summary

We are seeking a highly organized and detail-oriented individual to join our team as a Patient Pathway Coordinator. In this role, you will play a critical part in ensuring that patients receive timely and effective care.

Main Responsibilities
  • Proactively manage elective patient pathways to ensure compliance with the OUH Elective Access Policy and 18 weeks referral to treatment (RTT) rules.
  • Contribute to the validation of the PTL and investigate incomplete pathways to ensure patients are treated in clinical priority and breach date order.
  • Ensure trust systems are updated with patient pathway status information and maintain data quality.
  • Liaise with internal and external colleagues to share patient pathway information and expedite patient journeys.
  • Provide a robust administration function that underpins the delivery of a high-quality service and maintain effective working relationships with clinical, nursing, and administrative staff.
Requirements
  • Demonstrate high levels of customer care and be an ambassador for customer care within the trust.
  • Maintain patient confidentiality at all times.
  • Be flexible in your approach to work, including covering other job roles at an appropriate grade or site, and varying working hours to ensure the service maintains a minimum level of cover during core hours.
