Nurse Specialist

We are seeking a highly skilled and compassionate Registered Nurse to join our Clinical Co-ordination Centre team. As a key member of our multi-disciplinary team, you will play a vital role in providing specialist palliative nursing care and clinical advice to patients, carers, and healthcare professionals.
Main Responsibilities:- Assess and review patients to identify appropriate palliative and end-of-life care, ensuring safe and effective treatment and support.
- Work collaboratively with the multi-disciplinary team to ensure effective communication and patient care.
- Support junior colleagues in providing a consistent approach to the triage process, prioritizing patients appropriately.
- Adhere to Hospiscare's clinical record-keeping policy, maintaining patient confidentiality at all times.
- Attend and contribute to multi-disciplinary team meetings and case discussions to inform decisions that ensure patients receive the best possible care.
- Develop knowledge and skills to contribute to the delivery of education and learning for those involved in palliative/end-of-life care.
- Contribute to Hospiscare's clinical governance and audit process to support continuing quality improvement and learning.
- Develop advanced skills in identifying and assessing complex bereavement to enable a seamless handover into Hospiscare's Supportive Care Services.
- Registered Nurse with current Nursing and Midwifery Council registration.
- Evidence of continuing professional development.
- Post-registration palliative care module or equivalent.
- Teaching and mentorship/supervision qualification.
- Highly developed communication and interpersonal skills.
- Developed ability to manage a caseload effectively.
- Knowledge of theoretical nursing models and their impact on delivery of care.
- Understanding of key issues relating to End of Life care, including policy developments.
