Job Title: Registered Nurse

We are seeking a highly skilled and compassionate Registered Nurse to join our Clinical Co-ordination Centre team. As a key member of our multi-disciplinary team, you will play a vital role in providing specialist palliative nursing care and clinical advice to patients, carers, and healthcare professionals.

Main Responsibilities:
  • Assess and review patients to identify appropriate palliative and end-of-life care, ensuring safe and effective treatment and support.
  • Work collaboratively with the multi-disciplinary team to ensure effective communication and patient care.
  • Support junior colleagues in providing a consistent approach to the triage process, prioritizing patients appropriately.
  • Adhere to Hospiscare's clinical record-keeping policy, maintaining patient confidentiality at all times.
  • Attend and contribute to multi-disciplinary team meetings and case discussions to inform decisions that ensure patients receive the best possible care.
  • Develop knowledge and skills to contribute to the delivery of education and learning for those involved in palliative/end-of-life care.
  • Contribute to Hospiscare's clinical governance and audit process to support continuing quality improvement and learning.
  • Develop advanced skills in identifying and assessing complex bereavement to enable a seamless handover into Hospiscare's Supportive Care Services.
Requirements:
  • Registered Nurse with current Nursing and Midwifery Council registration.
  • Evidence of continuing professional development.
  • Post-registration palliative care module or equivalent.
  • Teaching and mentorship/supervision qualification.
Skills and Attributes:
  • Highly developed communication and interpersonal skills.
  • Developed ability to manage a caseload effectively.
  • Knowledge of theoretical nursing models and their impact on delivery of care.
  • Understanding of key issues relating to End of Life care, including policy developments.
