Palliative Care Centre Coordinator

We are seeking a skilled Palliative Care Coordinator to join our team at St Barnabas Community Services. As a key member of our Palliative Care Coordination Centre, you will play a vital role in coordinating care for patients wishing to remain at home.
Key Responsibilities:
- Liaise with NHS, voluntary, and private sector care providers to arrange packages of care for palliative patients.
- Use a telephone network and bespoke computer database to coordinate care.
- Work collaboratively with a team to ensure seamless care delivery.
Requirements:
- GCSE standard in Maths and English or NVQ/life skills equivalent.
- Computer literacy to CLAIT/ECDL standard or equivalent.
- Proven administration skills and experience in a multi-functional office environment.
Contract and Benefits:
- Fixed-term contract for 6 months.
- 30 hours per week, Monday to Friday.
- Salary: £24,048 per annum, pro rata (plus contributory pension scheme or continuation of NHS pension).
Working Arrangements:
- Working from home with occasional training in an office setting in Lincoln.
