Advanced Paramedic Practitioner

We are seeking an experienced Advanced Paramedic Practitioner to join our team in providing high-quality care to frail and older patients.
About the RoleThis is an exciting opportunity to work as part of a multi-disciplinary team, providing face-to-face and telephone consultations with patients presenting with a wide range of minor illnesses, urgent and same-day requests for treatment, and chronic disease management.
You will be responsible for assessing patients' medical conditions, providing care and treatment, and evaluating the effectiveness of care. A significant portion of your role will involve working with frail and elderly patients, including those in care and nursing homes.
As an Advanced Paramedic Practitioner, you will work closely with other healthcare professionals, including GPs, Advanced Nurse Practitioners, Nurse Practitioners, Clinical Pharmacists, and Physiotherapists, to provide comprehensive care to patients.
Key Responsibilities- Provide high-quality, cost-effective care to patients, focusing on frail and older people, and those with low-level mental health concerns.
- Work as part of a multi-disciplinary team to provide face-to-face and telephone consultations with patients.
- Assess patients' medical conditions, provide care and treatment, and evaluate the effectiveness of care.
- Work with frail and elderly patients, including those in care and nursing homes.
- Collaborate with other healthcare professionals to provide comprehensive care to patients.
- Full and unrestricted NMC (UK) or HCPC registration.
- RGN, RMN, or Paramedic qualification (minimum degree or equivalent, ideally master's level).
- Qualification in Physical Assessment skills.
- Prescribing Qualification.
- A qualification in Frailty Care.
