We are seeking a highly skilled and experienced Advanced Nurse Practitioner to join our team at Hastings & Rother Healthcare. As a key member of our multidisciplinary team, you will be responsible for providing high-quality, patient-centered care to frail and older patients in care homes and the community.
About the RoleAs an Advanced Nurse Practitioner, you will be working closely with our GP Partners, GPs, Advanced Clinical Practitioners, Nurse Practitioners, Paramedic Practitioners, Clinical Pharmacists, Physiotherapists, Practice Nurses, and Healthcare Assistants to provide comprehensive care to our patients. Your responsibilities will include:
- Providing face-to-face and telephone consultations with patients presenting with a wide range of minor illnesses, urgent, and same-day requests for treatment and chronic disease management.
- Assessing patients' medical conditions and frailty reviews, including visiting patients in their homes.
- Working collaboratively with our multidisciplinary team to provide high-quality, cost-effective care to our patients.
To be successful in this role, you will need:
- Full and unrestricted NMC (UK) or HCPC registration.
- RGN (degree or equivalent) qualification.
- Prescribing qualification.
- Qualification in Physical Assessment skills and Frailty.
- Qualification in Frailty Care.
