Job Title: Advanced Nurse Practitioner for Care Homes and Community Care

We are seeking a highly skilled and experienced Advanced Nurse Practitioner to join our team at Hastings & Rother Healthcare. As a key member of our multidisciplinary team, you will be responsible for providing high-quality, patient-centered care to frail and older patients in care homes and the community.

About the Role

You will work closely with our GP Partners, GPs, Advanced Clinical Practitioners, Nurse Practitioners, Paramedic Practitioners, Clinical Pharmacists, Physiotherapists, Practice Nurses, and Healthcare Assistants to provide comprehensive care to patients with a wide range of medical issues.

Key responsibilities include:

  • Providing face-to-face and telephone consultations with patients presenting with minor illnesses, urgent, and same-day requests for treatment and chronic disease management.
  • Assessing patients' medical conditions and frailty reviews in their homes.
  • Working with patients, families, and carers to develop personalized care plans.
  • Collaborating with other healthcare professionals to ensure seamless care transitions.
Requirements

To be successful in this role, you will need:

  • Full and unrestricted NMC (UK) or HCPC registration.
  • RGN (degree or equivalent) qualification.
  • Prescribing qualification.
  • Qualification in Physical Assessment skills and Frailty.
  • Qualification in Frailty Care.
