Hastings & Rother Healthcare is seeking a highly skilled Advanced Paramedic Practitioner to join our team in providing high-quality care to frail and older patients. As a key member of our multidisciplinary team, you will be responsible for delivering patient-centered care, working closely with GPs, Advanced Nurse Practitioners, and other healthcare professionals.
About the RoleThis is an exciting opportunity to work in a dynamic and supportive environment, where you will have the chance to make a real difference in the lives of our patients. As an Advanced Paramedic Practitioner, you will be responsible for:
- Providing high-quality, cost-effective care to patients with complex needs
- Working collaboratively with the multidisciplinary team to deliver patient-centered care
- Conducting face-to-face and telephone consultations with patients presenting with a wide range of medical issues
- Developing and implementing care plans to meet the individual needs of patients
- Visiting patients in their homes to assess their medical condition and frailty review
To be successful in this role, you will need:
- Full and unrestricted NMC (UK) or HCPC registration
- RGN, RMN or Paramedic qualification (minimum degree or equivalent, ideally masters level)
- Qualification in Physical Assessment skills
- Prescribing Qualification
- A qualification in Frailty Care
