About the Role

We are seeking a highly motivated and confident individual to join our team as a Community Access Practitioner. This role will involve working with adult males within HMP Full Sutton or Millsike who present with risky behaviours and display signs of mental health difficulties.

The successful candidate will oversee prisoner pathways from reception into the prison estate through to release, forging working alliances with local inpatient and community partners to develop effective reintegration pathways for patients.

As a Community Access Practitioner, you will develop strong therapeutic working relationships with service users, offering consistent engagement from initial referral to an identified Mental Health/LD provision.

You will also develop robust working relationships with local and partner agencies and services across the statutory, voluntary, and independent sectors, including MAPPA, Private Providers, and Local Authority Departments.

The post holder will deliver high-quality care to a caseload of complex mental health and learning disability offenders as part of a liaison and in-reach provision.
