About the Role:

We're seeking a Positive Intervention Practitioner/Social Care Worker to join our team at Bramerton, a large detached house supporting up to 11 male residents with moderate to severe learning disabilities and associated complex needs. As a member of our team, you'll play a vital role in providing person-centred care and support, promoting independence, and enhancing the quality of life for our residents.

Key Responsibilities:
  • Provide personal care, including bathing, toileting, dressing, and help with feeding
  • Follow care plan guidance and company policies and procedures
  • Attend and complete training as required
  • Assist in the development of interventions to reduce challenging behaviours
  • Offer individual and group work to residents under the supervision of the Clinical Lead and/or Senior Practitioner
  • Facilitate the smooth implementation of care plan systems within the homes
