About the Role

We are seeking a highly motivated and compassionate Support Practitioner to join our Blantyre Housing Support Service. As a Support Practitioner, you will work closely with an individual who requires 24-hour support due to complex health and communication needs. Your role will involve providing person-centred care, supporting the individual to engage in activities of their choice, and developing a deep understanding of their unique needs and preferences.

Key Responsibilities
  • Provide high-quality person-centred care to the individual, ensuring their physical, emotional, and social needs are met.
  • Support the individual to engage in activities of their choice, such as rebound sessions, hydrotherapy, and storytelling sessions.
  • Develop and maintain a deep understanding of the individual's unique needs and preferences, including their communication style and support requirements.
  • Work collaboratively with other health professionals and support departments to ensure the individual receives comprehensive care.
  • Drive the individual's vehicle on their behalf, ensuring safe and reliable transportation.
  • Provide personal care, support with finances, and administration of medication as required.
