About the RoleWe are seeking a highly motivated and compassionate Support Practitioner to join our Blantyre Service. As a Support Practitioner, you will play a vital role in providing person-centred care to an individual with complex health needs. Your primary responsibility will be to support the individual in their daily life, including personal care, finances, and administration of medication.
Key Responsibilities- Provide high-quality, person-centred care to the individual, respecting their needs and preferences.
- Develop and maintain a strong relationship with the individual, their family, and other stakeholders.
- Work collaboratively with the multidisciplinary team to ensure seamless care and support.
- Participate in regular supervision and team meetings to discuss progress and challenges.
- Complete all relevant training and eLearning modules to enhance your skills and knowledge.
- Good team-working skills and a positive attitude.
- Ability to communicate effectively and use your own initiative.
- Strong work ethic and organised to support routines.
- Able to prioritise and manage workloads.
- Eager to develop and learn.
- A genuine passion for caring for others.
- Patience and understanding.
