Advanced Care Practitioner

We are seeking an experienced Advanced Care Practitioner, Prescribing Nurse Specialist, or Paramedic Clinician to join our team at Eden Medical Group. As a key member of our Primary Care Network, you will play a vital role in delivering high-quality patient care and contributing to the development of our innovative services.
Key Responsibilities- Provide clinical leadership and expertise in the assessment, diagnosis, and management of patients with complex health needs.
- Develop and implement evidence-based care plans, working collaboratively with multidisciplinary teams to ensure seamless patient care.
- Prescribe medications and treatments, ensuring safe and effective use of medications and minimizing polypharmacy.
- Conduct home visits, providing patient-centered care and promoting health and wellbeing.
- Collaborate with the Frailty Team to provide comprehensive care to patients with frailty and complex needs.
- Participate in the development and delivery of new services, contributing to the improvement of patient outcomes and experiences.
- Current professional registration as an Advanced Care Practitioner, Prescribing Nurse Specialist, or Paramedic Clinician.
- Independent Non-Medical Prescriber V300 qualification.
- Evidence of recent Continual Professional Development.
- Leadership skills and experience in managing complex patient care.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to assess and manage patient risk effectively and safely.
- Flexibility and adaptability, with a willingness to learn and develop new skills.
