Position Overview
The primary objective of this role is to deliver guidance, support, and resources to empower young individuals in optimizing their educational, vocational, and housing prospects.
It is essential to ensure that these young individuals receive timely and appropriate assistance to facilitate their transition into adulthood and independent living.
Responsibilities include:
- Contributing to the evaluations of care leavers and their families in need of support.
- Providing necessary assistance under the guidance of a Senior Practitioner or Team Manager.
- Identifying deficiencies in the leaving care services and proposing enhancements to improve service delivery.
- Participating in the formulation of relevant policies and procedures, including referral criteria and recording protocols, to ensure clarity for families and local agencies regarding the role of Children's Social Care with Care Leavers.
- Actively engaging as a team member and attending meetings to collaboratively review work and manage caseloads with the Team Manager.
