About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at Active Care Group. As a Support Worker, you will play a vital role in providing high-quality care and support to adults with complex needs in their own homes.

Key Responsibilities
  • Provide personal care and support to clients with spinal cord injury, acquired brain injury, and complex health needs
  • Assist with medication management, moving and handling, and managing clinical equipment
  • Work collaboratively with the Care Manager and other healthcare professionals to ensure seamless care delivery
  • Develop and maintain strong relationships with clients and their families to ensure their needs are met
