We are seeking a highly motivated and compassionate Support Worker to join our team at Evolve Housing + Support. As a Support Worker, you will play a vital role in providing person-centred support to vulnerable individuals, empowering them to regain their independence and live fulfilling lives.
Key Responsibilities- Identify and support customers' strengths, needs, and goals, working collaboratively to develop and implement personalized support plans.
- Provide emotional and practical support, promoting customer involvement and co-production in all aspects of their lives.
- Establish and maintain supportive relationships with customers, colleagues, and external partners, ensuring effective communication and collaboration.
- Contribute to the development and implementation of customer-centered initiatives, promoting positive outcomes and empowering customers to achieve their potential.
- Participate in ongoing training and professional development, staying up-to-date with best practices and industry developments.
