We are seeking a highly motivated and compassionate Support Worker to join our team at Evolve Housing + Support. As a Support Worker, you will play a vital role in providing person-centred support to vulnerable individuals, empowering them to regain their independence and achieve their goals.
Key Responsibilities- Identify and support customers' strengths, needs, and aspirations, and develop personalized plans to achieve their goals.
- Provide emotional and practical support to customers, promoting their confidence, self-esteem, and independence.
- Work collaboratively with customers, colleagues, and external partners to ensure seamless support and services.
- Conduct risk assessments and develop management plans to ensure customers' safety and well-being.
- Support customers to access benefits, housing, education, and other essential services.
- Network and build relationships with external agencies to ensure customers receive comprehensive support.
- Participate in training and development opportunities to enhance your skills and knowledge.
- Knowledge of safeguarding principles and practices.
- Understanding of the reasons why people become homeless and the needs and challenges they face.
- Ability to work shifts, including mornings, evenings, and weekends.
