Job Title: Band 5 Community Nursing
Job Summary:
We are seeking a highly motivated and experienced Community Nursing Specialist to join our team at Birmingham Community Healthcare NHS Foundation Trust. As a Community Nursing Specialist, you will be responsible for providing high-quality nursing care to patients in their own homes, GP premises, and other community settings.
Main Responsibilities:
- Work as part of an integrated multidisciplinary team to provide holistic care to patients with a range of health needs, including palliative care, wound management, diabetes, and other long-term conditions.
- Conduct assessments and evaluations of patients' health needs, and develop and implement individualized care plans to promote independence and well-being.
- Work closely with other members of the multidisciplinary team, including GPs, Rapid Response, and Advanced Nurse Practitioners, to prevent inappropriate hospital admissions and facilitate hospital discharges.
- Provide education and support to patients and their families to promote health and well-being.
- Participate in the development and implementation of service objectives and quality improvement initiatives.
Requirements:
- Registered Nurse Level 1 (Adult) qualification.
- Mentor and assessor qualification or equivalent.
- Experience of working in a multi-disciplinary team setting.
- Ability to demonstrate sound knowledge and application of up-to-date evidence-based clinical practice.
- Effective time management and communication skills.
- Ability to work under pressure and as part of a team.
Working Arrangements:
The successful candidate will be required to work across various sites in the west locality of Birmingham, and will need to be independently mobile with access to a car. The service operates between the hours of 8 am to 6 pm over 7 days per week, and the working pattern will include weekend working and bank holiday periods.
