About the Role

We are seeking a highly skilled and compassionate Recovery Outreach Worker to join our Mental Health and Complex Needs Service at Hestia. As a key member of our team, you will play a vital role in supporting individuals with severe and enduring mental health needs, promoting recovery, and preventing social exclusion.

Key Responsibilities
  • Provide holistic, person-centered support to a caseload of service users, including those from African, Caribbean, and Arabic speaking heritage, to increase independence, maintain accommodation, and enjoy a good quality of life.
  • Develop and implement effective interventions for service users who are 'hard to engage', ensuring they access medical services and local community mainstream services.
  • Conduct regular needs and risk assessments, SMART support planning, and reviews to minimize unplanned hospital admissions and promote recovery.
  • Liaise with other agencies, advocate on behalf of service users, and encourage engagement with health and social networks, including faith-based groups and informal sources of support.
Requirements
  • Demonstrable experience of working with people from diverse backgrounds, particularly African, Caribbean, and Arabic heritage, requiring support related to mental ill health and complex needs within a community or outreach setting.
  • Ability to support service users to achieve their goals and aspirations in a culturally appropriate setting.
  • Excellent knowledge of mental health issues, including signs, symptoms, and treatments, with the ability to assess declining mental health and take appropriate action.
  • Experience of promoting, implementing, and maintaining Equality, Diversity, and Inclusion good practice.
  • Knowledge of health and safety relevant to the role, including lone working within a community setting.
