The Community Support Specialist will work as part of a multi-disciplinary team to support the delivery of recovery care plans for clients with a range of mental health presentations in the community and facilitating discharges from hospital. A key role will be to support the team in enabling safe administration of Electroconvulsive Therapy (ECT) in Primary Care, Complex Care Teams, and Third sector organizations.

The post holder will work intensively with individuals to support them in their homes, preventing hospital admissions and facilitating hospital discharges. They will also have part of their time planned for specific activities, including linking with wards to support patients to go out while they are inpatients, supporting the assessment of carers as part of a Devon Memory Service assessment, and supporting the OPMH functional wards to ensure safe transfer and administration of ECT to patients while they are inpatients.

The Community Support Specialist will work to a clearly defined personal recovery plan outlining the role for the post holder with each client and the range of interventions required.

Main Responsibilities
  1. Provide support to older adults in their own homes, delivering practical help and advice where possible.
  2. Consider community support available and link in where possible.
  3. Support individuals in escorting them to ECT.
  4. Support the Devon Memory Service, with training provided.
  5. Hold a UK driving license.
