    • Develop and maintain effective relationships with service users, carers, and other professionals to ensure seamless care coordination.
    • Communicate complex information in a clear and concise manner, taking into account individual needs and preferences.
    • Participate in and support positive interventions and planned activity programs to improve service users' quality of life and mental state.
    • Foster and maintain good working relationships with community teams, health and social care workers, and other stakeholders.
  • Analytical and Judgemental Skills
    • Assess and manage risk in conjunction with qualified staff, ensuring the safety and well-being of service users.
    • Take part in health and safety matters as appropriate, adhering to Trust policies and procedures.
    • Use community resources effectively to support service users' needs.
  • Planning and Organisational Skills
    • Plan and prioritize your own workload, maintaining accurate records of all activities.

    • Recognize signs of abuse and know procedures to follow to escalate concerns.
    • Participate in and support positive interventions and planned activity programs to improve service users' quality of life.
    • Record contacts, interventions, and observations on the electronic care record, seeking professional validation as needed.
  • Responsibility for Policy and Service Development Implementation
    • Comply with all DPT policies, protocols, and procedures relevant to the role.
    • Report any observed incidents of unprofessional behavior, accidents, or untoward occurrences.
    • Participate professionally in service development and proposed service changes.
  • Responsibility for Finance, Equipment, and Other Resources
    • Assist in maintaining safety and security of the environment.
    • Help maintain cleanliness and tidiness of the environment, including equipment.
    • Be aware of counter-fraud procedures and adhere to Trust policies on financial issues.
  • Responsibility for Human Resources, Supervision, Training, HR Advice, and Management
    • Work under daily supervision from a mental health practitioner, supporting service users in their own homes and community settings.
    • Participate in regular supervision and annual appraisal, adhering to Trust policy.
  • Responsibility for Information Resources and Administrative Duties
    • Maintain up-to-date electronic records in a timely and accurate manner.
    • Regularly access email and other information through the Trust's intranet.
    • Maintain confidentiality of information, adhering to Trust policies and local protocols.
