About the Role

The Royal Devon and Exeter NHS Foundation Trust is seeking a skilled Community Matron to join their team. As a Community Matron, you will play a pivotal role in providing healthcare to patients in their own homes, preventing hospital admissions, and facilitating early discharge from the hospital.

Key Responsibilities

  • Assess and manage long-term conditions
  • Develop professional relationships with medical colleagues, other health professionals, social services, and voluntary agencies
  • Facilitate discharge of patients through the system
  • Support patients with complex needs and chronic disease
  • Participate in the Senior Clinician on-call rota

Requirements

  • Minimum of degree-level education
  • V prescribing qualification or Specialist Nurse Practitioner qualification
  • Ability to work independently and as part of a team
