About the Role

We are seeking a dedicated and compassionate Support Practitioner to join our team in Dunoon & Cowal. As a Support Practitioner, you will play a vital role in providing immediate support to individuals in our community, ensuring their well-being and safety.

Key Responsibilities
  • Conduct personal care and practical support to individuals in need
  • Perform dynamic risk assessments and screen needs through effective communication
  • Work proactively to prevent hospital admissions and promote holistic care
  • Liaise with partner organisations to ensure continuity of care
