About the Role:

Carr Gomm is a leading social care and community development charity. We are seeking a highly motivated and compassionate individual to join our team as a Support Practitioner in Dunoon and Cowal. As a Support Practitioner, you will be the essential link in providing immediate support in urgent situations, ensuring the well-being and safety of people in our community.

Key Responsibilities:

  • Conduct personal care and practical support to individuals in their own homes
  • Perform dynamic risk assessments and screen needs through effective communication
  • Work proactively to prevent hospital admissions and promote holistic care and wellbeing
  • Liaise with partner organisations to contribute to the care and wellbeing of individuals

Requirements:

  • Caring and compassionate individual with excellent communication skills
  • Ability to remain calm under pressure and work independently
  • Full driving license and willingness to travel
