Job Title: Patch Children's Community Nurse

We are seeking a highly skilled and experienced Children's Community Nurse to join our team at Guy's and St Thomas' NHS Foundation Trust. As a key member of our Patch Children's Community Nursing (CCN) team, you will play a vital role in providing high-quality care to children and young people with ongoing conditions such as asthma, eczema, and constipation.

About the Role

This is an exciting opportunity to work in a dynamic and innovative team that is making a difference in children's lives through community-based care. As a Children's Community Nurse, you will be responsible for assessing, planning, and delivering specialist nursing care to children and young people in various settings, including homes, schools, and community clinics.

You will work collaboratively with our multidisciplinary team to provide expert nursing advice, carry out specialized nursing procedures, and contribute to ongoing service development and integration of care into the wider children's community nursing service.

Key Responsibilities
  • Facilitate early discharge and reduce hospital length of stay
  • Prevent hospital admissions by delivering proactive care in the community
  • Provide clinical supervision to staff and students
  • Engage in research and lead clinical audits within your specialist area
Requirements

To be successful in this role, you will need to have:

  • Experience of working as an autonomous practitioner within acute, community, or primary care settings
  • Excellent organisational, time management, and communication skills
  • Demonstrated awareness of clinical governance and risk management
  • Effective technical skills and ability to use IT systems to manage patients
  • Experience of teaching, assessing, and supervising others in a clinical setting
