Position Overview

An exceptional opportunity has emerged for qualified and experienced professionals to become part of the Camden Adult Pathway Partnership (CAPP). This initiative represents a collaborative effort between UCLH and local health services, focusing on delivering healthcare solutions to vulnerable populations.

We are in search of dedicated and enthusiastic individuals who will play a vital role in providing comprehensive healthcare services for individuals experiencing homelessness within the Camden Hostel Pathway. The role involves collaborating closely with the Head of Integrated Operations to foster effective partnerships with both general and specialized clinical teams, hostel administrators, and various outreach services within the integrated care framework, both internally and externally to UCLH, to ensure optimal healthcare delivery to residents of the Camden hostel pathway.

Key Responsibilities

Senior Healthcare Coordinator for Community Homeless Services
We are seeking a seasoned healthcare professional capable of working innovatively and flexibly alongside the Clinical Nurse Specialist and project lead to guarantee the provision of high-quality healthcare services.

This position is on a fixed-term basis, with potential for extension subject to funding confirmation.
