Position Overview
Care Plus Group is currently seeking a dedicated and experienced professional to join our Community Urgent Care Team as a Lead Community Support Specialist. This role is pivotal in delivering a comprehensive health and social care triage service, aimed at preventing unnecessary hospital admissions and facilitating timely discharges.
Key Responsibilities
As a Lead Community Support Specialist, your primary responsibilities will include:
- Conducting thorough assessments of complex needs to formulate, implement, and evaluate tailored care programs.
- Supporting individuals in maintaining their independence within community settings, including their own homes or care facilities.
- Engaging in multi-disciplinary assessments and contributing to the discharge planning process.
- Completing assessments in accordance with the Care Act 2014 and participating in best interest discussions.
- Providing guidance and support to team members and collaborating with various stakeholders to ensure optimal care delivery.
Work Schedule
This is a permanent position requiring a commitment to a flexible rota pattern, including 12-hour shifts across weekdays, weekends, and public holidays.
Compensation
The salary range for this position is competitive, reflecting experience and qualifications.
Qualifications
To be considered for this role, candidates must possess:
- A relevant social work qualification at the degree level.
- Current registration with the appropriate professional body.
- Experience in working with vulnerable adults and a solid understanding of relevant legislation and policies.
