About the Role

We are seeking a highly skilled and dedicated Community Mental Health Nurse to join our team at Essex Partnership University NHS Foundation Trust. As a key member of our nursing team, you will provide high-quality, evidence-based care to patients in line with the NMC professional code of conduct and Trust policies.

Main Responsibilities
  • Provide and oversee the management and care of service users, ensuring their needs are met and their safety is prioritized.
  • Conduct risk assessments and develop care plans to mitigate potential hazards and promote a safe environment.
  • Administer medications, monitor side effects, and review medication regimes to ensure optimal clinical effectiveness.
  • Act as a care coordinator and key worker, providing education and support to service users and their families.
  • Collaborate with multidisciplinary teams to ensure seamless care delivery and achieve agreed performance targets.
Requirements
  • Valid UK driving license and access to a car for work purposes.
  • Experience working in community mental health settings, with a strong understanding of NICE guidelines and Trust policies.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
