About the Role

We are seeking a skilled and compassionate Clinical Support Worker to join our Reablement Team at Norfolk Community Health and Care NHS Trust. As a Clinical Support Worker, you will play a vital role in supporting patients to regain their independence and confidence in their daily lives.

Key Responsibilities
  • Work as part of a multidisciplinary team to deliver high-quality care to patients in a reablement setting.
  • Support patients to achieve their goals and maintain their independence through a range of activities, including washing, dressing, and nutritional needs.
  • Collaborate with healthcare professionals to develop and implement individualized care plans that promote patient-centered care.
  • Provide emotional support and encouragement to patients, promoting a positive and empowering environment.
  • Document patient care and progress accurately and confidentially, using electronic systems as required.
  • Respond to clinical emergencies and provide appropriate care, adhering to trust policies and procedures.
Requirements
  • Education to GCSE level Grade 5 in English and Mathematics or equivalent.
  • Care Certificate and/or equivalent healthcare experience.
  • Some previous experience in a nursing or care assistant role.
  • Good organizational skills, basic IT and computer skills, and good interpersonal and communication skills.
  • Clear oral communication and a caring personality.
