We are seeking an experienced and skilled Senior Case Manager to join our team at Croydon Health Services NHS Trust. As a key member of our Integrated Care Team, you will play a vital role in developing and implementing integrated neighbourhood/care teams to improve patient outcomes and experiences.
Key Responsibilities- Operational Management and Clinical Leadership: Provide operational management and professional clinical leadership for INT functions, ensuring seamless delivery of services and high-quality patient care.
- Performance Oversight: Oversee the performance of core teams, delivering against agreed objectives and target outcomes within allocated resources.
- MDT Coordination: Coordinate the formation of multidisciplinary teams, delivering care via MDT meetings and enabling closer working across Health and Care Localities teams and Primary Care Networks.
- Innovation and Change Management: Work closely with the Integrated Management Team and existing service leads to test and embed innovative solutions to transform service delivery and improve patient outcomes.
- Quality Improvement: Ensure the quality of patients' experience and lead on facilitating changes designed to improve patient experience.
- Care Pathway Development: Support the development of care pathways that deliver on partner objectives and ensure excellent patient outcomes and experience.
- Education and Qualifications: Education to first level degree or equivalent, Independent prescriber V300, Enhanced Physical Assessment Skills and history taking, Current NMC/HCPC registration, and Car driver and owner.
- Experience: Evidence of post-registration education, Primary Care experience in care of people with long-term conditions, Experience of working in Community services, Experience of clinical audit, benchmarking, and evaluation of services, and Experience of interagency and collaborative working.
- Skills: Report writing skills, including presentation of data, Excellent IT, written, and communication skills, Excellent time management skills, and Ability to influence and lead change at senior level.
- Personality and Personal Attributes: Able to work effectively in different situations, with groups or individuals exhibiting a range of competences, behaviours, and motivations, Able to investigate, de-escalate, and resolve issues that may be sensitive or difficult, and Experience of customer service in a multi-cultural environment.
- Knowledge and Understanding: Knowledge of current community service changes and direction, Knowledge of current local priorities and developments within the community setting, Knowledge of clinical risk strategies, and Good working knowledge of wider health and social care system and structures, policies, procedures, and legislation relevant to services areas.
