Job Description

Job Title: Community Nutrition Specialist

Job Summary:

We are seeking a highly skilled and experienced Dietitian to join our Community Nutrition and Dietetics team in Central Lancashire. As a Community Nutrition Specialist, you will play a key role in providing high-quality nutrition services to patients in clinic, in their own homes, and in nursing and care homes.

Main Responsibilities:

  • Assess and implement care plans for patients in clinic, in their own homes, and in nursing and care homes.
  • Provide nutrition education and training to healthcare professionals and carers.
  • Develop and maintain IT systems that support patient care.
  • Collaborate with other healthcare professionals to provide comprehensive care to patients.

Requirements:

  • GCSE or equivalent in Maths and English.
  • Degree in Nutrition/Dietetics or equivalent leading to Health Professions Council (HCPC) Registration as a Dietitian.
  • Teaching qualification or equivalent/demonstration of teaching/supervision experience.
  • Compliance with regulatory bodies and codes of professional conduct.
  • Knowledge of current key NHS developments and policies and implications for service delivery.

Desirable Qualifications and Experience:

  • Experience of partnership working with other agencies and professional boundaries.
  • Experience in leading the change processes at an operational level.
  • Experience of effectively working with service users and carers.
  • Experience of clinical professional teaching in a community healthcare setting.
  • Experience working as an independent and autonomous practitioner.
  • Experience of standard setting.
  • Evidence of management Development through continuous professional Development and/or formal Management courses.
  • Participation and leading clinical audit within own area.
  • Participation on clinical supervision mentorship.
