Job Summary

This role offers a unique opportunity to lead in providing top-notch care and support to cancer patients and their families. You will play a crucial part in expanding and strengthening existing efforts to enhance care for colorectal cancer patients as part of our post-treatment program for cancer survivors.

Key Responsibilities

  • Provide specialized nursing advice supported by theory and experience to assist patients and their families/carers.
  • Serve as a specialist practitioner, devising, implementing, and evaluating care programs in collaboration with senior colleagues through comprehensive assessment, personalized care planning, referrals, and discharge plans to primary care units.
  • Offer specialist advice to manage complex symptoms for patients and staff.
  • Develop and share expertise within the multidisciplinary team.
  • Collaborate with various healthcare teams to ensure a seamless and effective service delivery.
  • Deliver presentations at the colorectal weekly Multidisciplinary team meetings held at the Southmead site.
  • Utilize advanced skills to facilitate effective communication support for patients, including handling challenging conversations and delivering sensitive information.
