Position Overview

An exceptional opportunity has arisen to become a vital member of a proactive team dedicated to guiding patients through the cancer diagnostic journey.

The Rapid Diagnostic Centre/Straight to Test initiative is a nurse-led program that oversees urgent referrals from Lower Gastrointestinal and Non-Site Specific Upper Gastrointestinal cases.

We are currently offering three full-time equivalent positions, with two focused on Colorectal and one on Upper GI. While all roles are Trustwide, one Colorectal position is based at Scunthorpe General Hospital and the other at Diana, Princess of Wales Hospital.

As a Pathway Coordinator, you will play a crucial role in ensuring that patients receive timely appointments and necessary investigations, while also meeting cancer care targets. You will serve as a primary contact for patients throughout their journey, addressing inquiries and providing updates on their investigation outcomes.

Key Responsibilities

  • Oversee the daily management of all colorectal two-week wait referrals received by the Trust. This includes assessing all referrals to direct them to the appropriate clinics based on patient needs, contacting patients to introduce the service, scheduling appointments, and collaborating with the Clinical Nurse Specialists (CNS) and other team members to manage each patient's care pathway.
  • Assist patients through the rapid diagnostic centre and colorectal straight to test pathways.
  • Evaluate incoming calls using a risk assessment framework, providing appropriate responses in accordance with established protocols and individual care pathways, utilizing effective communication skills and basic clinical knowledge, while liaising with CNS as necessary.
  • Utilize sound judgment to address the needs of individuals, identifying the complexity of situations, offering suitable advice, and escalating issues to registered practitioners when required.

Qualifications and Experience

Essential
