Job Summary

The Beacon Medical network is seeking a compassionate and dedicated Frailty Care Coordinator to join our team. In this role, the Frailty Care Coordinator will play a crucial administrative role, collaborating with healthcare providers and community care teams to ensure that frail patients receive appropriate interventions, aimed at assisting them in maintaining as much independence as possible.

This entails coordinating various clinical and social care services, as well as assisting individual patients in identifying and accessing support services provided by healthcare providers and the voluntary sector.

Main Responsibilities

  • Reviewing patient data to proactively identify frailty and intervene before crisis.
  • Coordinating multidisciplinary teams between healthcare providers, community care teams, and social services to optimize patient care.
  • Liaising with healthcare providers and community care teams to identify issues and barriers, and being the main point of contact for patients and families.
  • Supporting discharges from hospital, creating robust processes and pathways between healthcare providers and community care teams.
  • Improving data quality coding of frail and housebound patients in electronic medical record systems.
  • Liaising with healthcare providers and community care teams to standardize patient care.
  • Working closely with clinical leads to ensure smooth working and training between healthcare providers and community care teams.
  • Facilitating referrals to healthcare services, being the main point of contact for patients and families.
  • Improving patient access to healthcare providers.
