Job Overview

Beacon Medical is actively seeking a dedicated and empathetic Frailty Care Coordinator to enhance our growing team. In this pivotal role, the Frailty Care Coordinator will be responsible for administrative functions, working in tandem with healthcare practices and the wider community care network to ensure that patients experiencing frailty receive timely and suitable interventions, promoting their independence.

This position involves orchestrating a variety of clinical and social care services, while also guiding patients in recognizing and utilizing support services offered by community organizations and healthcare providers.

Key Responsibilities

Assess the outcomes of home visits conducted by the Frailty Advanced Nurse Practitioner to proactively identify frailty and implement interventions before emergencies arise. Coordinate multidisciplinary teams, including Primary and Secondary Care, Hospice, and Care Home personnel, to collaborate and enhance patient care. Act as the primary liaison with frailty nursing staff to identify challenges and obstacles in patient care. Facilitate smooth discharges from healthcare facilities, establishing robust processes and pathways between various care sectors. Enhance the accuracy of data entry related to frail and housebound patients within the medical record systems. Collaborate with multiple GP practices to standardize care protocols for patients. Work closely with Clinical Leads to ensure effective collaboration and training between Care Homes and clinical staff. Manage referrals to healthcare services, serving as the main point of contact for patients. Assist in improving patient access to general practitioners.

Candidate Profile

Required Qualifications

Exceptional interpersonal and communication skills to effectively engage with patients, families, and healthcare teams. Strong organizational abilities to manage multiple responsibilities and prioritize tasks efficiently. Familiarity with electronic medical record systems and relevant software (training will be provided). A compassionate and understanding approach towards patients and their families. Capability to work independently as well as collaboratively within a team, even under pressure. A proactive mindset, with the ability to independently resolve issues. Responsibility for task management and progress, with the initiative to seek support when necessary. Ability to prioritize and manage various tasks effectively.

Preferred Qualifications

Experience in primary care or GP practice settings. Familiarity with SystmOne software. Understanding of frailty and its associated challenges. Background in care coordination.
