In this pivotal role, the Frailty Care Coordinator will undertake essential administrative responsibilities, working in partnership with healthcare practices and the wider community care network to ensure that frail patients receive timely and appropriate support, aimed at fostering their independence.
This position involves orchestrating a variety of clinical and social care services, as well as guiding individual patients in locating and utilizing support services offered by both voluntary organizations and healthcare providers.
Key Responsibilities
Assess the Frailty Advanced Nurse Practitioner's home visits to proactively identify frailty and implement interventions prior to a crisis. Organize multidisciplinary collaborations among Primary & Secondary Care, Hospice, and Care Home personnel to enhance patient care. Serve as the primary liaison with frailty nursing staff to address challenges and barriers. Facilitate hospital discharges, establishing robust processes and pathways between Primary, Secondary, and Community Care. Enhance the accuracy of data coding for frail and housebound patients within the GP medical system. Collaborate with all GP practices within the network to standardize patient care approaches. Work closely with Clinical Leads to ensure effective collaboration and training between Care Homes and clinical staff. Streamline referrals to healthcare services, acting as the main point of contact. Assist in improving patient access to general practitioners.About Beacon Medical

Essential Qualifications
Strong interpersonal and communication skills to effectively engage with patients, families, and healthcare teams. Excellent organizational abilities with a talent for managing multiple priorities. Proficiency in electronic medical record systems and relevant software (training will be provided). A compassionate and empathetic demeanor towards patients and their families. Capability to work autonomously and collaboratively within a team, even under pressure. A proactive approach to problem-solving, with the ability to work independently or seek assistance when necessary. Ability to manage and prioritize various tasks efficiently.Desirable Qualifications
Experience in primary care or GP practice settings. Familiarity with SystmOne software. Understanding of frailty and its associated challenges. Background in care coordination.
