GLL is seeking a dedicated GP Health Referral Facilitator to contribute to our mission of promoting health and well-being at Energise Leisure Centre. This role offers a unique opportunity to make a tangible impact within our York Partnership.
As the primary liaison between GLL and GPs, Health Professionals, and local authorities, you will play a pivotal role in the successful execution of the Healthwise program. Collaborating closely with a dynamic team, you will oversee all aspects of this initiative, ensuring its smooth operation and positive outcomes.
Your Responsibilities:- Effectively manage and coordinate the Healthwise Program, driving its success and achieving desired results.
- Serve as the primary point of contact for GPs, Health Professionals, and local authorities, fostering strong relationships and facilitating seamless communication.
- Execute both practical and administrative tasks associated with the Healthwise program, ensuring its efficient and effective implementation.
- Represent the Healthwise program in internal and external meetings, effectively communicating its goals, progress, and impact.
- Prepare comprehensive monthly, quarterly, and annual reports, providing insightful data on program performance and achievements.
- Possess a Level 3 GP Referral qualification, demonstrating your expertise in this specialized field.
- Hold an NVQ Level 2 Fitness Instructor certification, showcasing your understanding of fitness principles and practices.
- Be a REPS registered fitness professional with proven experience in delivering lifestyle interventions.
- Demonstrate patience, empathy, and sensitivity when interacting with individuals from diverse backgrounds.
