Salary Range: £38,092 to £45,389 per annum (inclusive of £3,000 per annum market supplement)

Position Summary:

As a key member of our multi-disciplinary Hospital Team, you will collaborate with both health and social care professionals to provide exceptional social work services that significantly impact adults requiring social support. As a fully qualified Social Worker with expertise in safeguarding, intricate case management, resource distribution, and funding, you will be dedicated to empowering individuals to achieve optimal outcomes—ensuring their safety and independence to lead fulfilling lives.

Key Responsibilities:

  1. Lead and oversee a team of Senior Case Workers and Social Workers while managing a small, yet complex, caseload.
  2. Support the overall team management and assist in strategic planning and service delivery in accordance with governance principles.
  3. Utilize your strong communication and relationship-building skills to foster effective and positive interactions with individuals and healthcare professionals, both within and outside the hospital team.
