About the Role

This is an exciting opportunity to join NHS Professionals Limited as a Community Health Nurse. As a key member of our team, you will be responsible for providing high-quality patient care in a community setting.

Key Responsibilities

  • Assess and monitor patient conditions, arranging diagnostic tests as necessary
  • Recognize and respond to deterioration in patient condition, taking appropriate action and escalating concerns when necessary
  • Manage emergency situations, displaying excellent analytical ability and sound decision-making skills
  • Develop and implement individualized care plans, promoting patient autonomy and independence
  • Collaborate with multidisciplinary teams to ensure seamless patient care

Requirements

  • A minimum of 6 months paid experience in a community environment within the last 2 years
  • Current registration with the Nursing and Midwifery Council (NMC)
  • A car and valid UK driving license
