About the Role
Livewell Southwest CIC is seeking a skilled Community Health Nurse to join our team. As a Community Health Nurse, you will be responsible for providing high-quality care to patients in their homes, working closely with other healthcare professionals to ensure seamless care.
Key Responsibilities
- Assess, plan, and deliver care to patients in their homes, including wound management, pressure ulcer prevention, and palliative care.
- Work collaboratively with other healthcare professionals, including GPs, therapists, and social care teams, to ensure comprehensive care.
- Develop and maintain relationships with patients, carers, and families to ensure person-centered care.
- Participate in health promotion activities and contribute to the development of new projects and research initiatives.
- Prescribe medications and treatments as needed, in line with Non-medical Prescribing policy and the Devon Formulary.
Requirements
- Registered Nurse with a valid NMC registration.
- Experience in community nursing, preferably in a District Nursing Service.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, carers, and healthcare professionals.
- Ability to travel across Plymouth and other geographical areas, with a full driving license and access to a car for work purposes.
- Flexibility and adaptability, with a willingness to work in a fast-paced environment.
