Job Summary:
Hunter AHP is seeking a skilled Physiotherapist to join our team as a Community Reablement Specialist. This is a fantastic opportunity to work in a dynamic and supportive environment, providing high-quality care to adults in need of rehabilitation and reablement services.
Key Responsibilities:
- Deliver general rehabilitation and reablement services to adults in a community setting
- Work collaboratively with a multidisciplinary team, including Occupational Therapists, Physiotherapists, and rehab assistants
- Conduct thorough assessments and develop personalized care plans to meet the unique needs of each patient
- Provide education and support to patients and their families to promote independence and self-management
- Participate in regular team meetings and contribute to the development of care pathways and protocols
Requirements:
- Registration with the relevant professional body (e.g. HCPC)
- Experience working in a community reablement setting
- Strong communication and interpersonal skills
- Ability to work autonomously and as part of a team
- Flexibility to work varying shifts and hours
