About the Role

We are seeking a dedicated and motivated Mental Health Practitioner to join our Assertive Outreach team in Weymouth. As a key member of our multi-disciplinary team, you will work closely with Consultant Psychiatrists, Psychologists, and other healthcare professionals to provide person-centred targeted support to patients with severe and enduring mental illness.

Main Responsibilities

  • Provide holistic recovery support to a caseload of patients, focusing on engagement and recovery philosophies.
  • Conduct regular clinics, including depot and Olanzapine administration, and physical health checks.
  • Develop and maintain strong relationships with service users, families, and carers.
  • Collaborate with the team to identify and address patient needs, ensuring comprehensive care coordination.
  • Participate in regular supervision and peer support to enhance clinical skills and knowledge.

Requirements

  • Valid driving licence and use of a car for patient assessments and monitoring.
  • Recent experience working in a mental health environment.
  • Excellent communication, organisational, and prioritisation skills.
  • Ability to work independently and as part of a team.
