Job Title: Band 5 Community Mental Health Practitioner

We are seeking a highly skilled and dedicated Mental Health Practitioner to join our well-established Community Mental Health Team in Weymouth & Portland.

About the Role

This is an exciting opportunity for a Registered Mental Health Nurse, Occupational Therapist, or Social Worker to work as an Assessment and Duty worker, providing specialist care and interventions to individuals with severe mental illness.

You will work closely with a multidisciplinary team to triage referrals, gather information, and arrange assessments. This role also involves taking telephone calls from people on Medic caseloads and CCOs when unavailable.

Main Duties
  • Provide a high-quality, person-centered approach to care delivery, considering individuals' safety, privacy, and dignity.
  • Deliver specialist care and interventions, assessments, and treatment as part of the wider multidisciplinary care team.
  • Offer specialist advice to others regarding patient/service user management and care.
  • Develop and implement specialist programs of care for other professionals to deliver.
  • Plan, implement, and review health improvement programs in various settings.
  • Provide supervision, mentorship, training, and clinical support to students and trained practitioners.
Person Specification

We are looking for a candidate with:

  • Registration as a Mental Health Nurse, Occupational Therapist, or Social Worker to degree/diploma level, supplemented by post-registration diploma-level specialist training and/or short courses.
  • Membership of the relevant Professional Body.
  • Evidence of recent professional development in an up-to-date portfolio.
Desirable Qualifications and Experience

We would welcome applications from candidates with:

  • Knowledge and understanding of Trust Strategy relevant to the role.
  • Recent previous experience within a comparable role.
  • Experience at Practitioner Band 5 level.
Additional Requirements

The successful candidate will be able to:

  • Travel using their own vehicle on Trust business.
  • Communicate effectively in written and spoken English to a standard that enables them to carry out the full range of duties and responsibilities of the role effectively.
