Job Summary

Dorset HealthCare University NHS Foundation Trust is seeking a dedicated Mental Health Practitioner to join our Assertive Outreach team in Weymouth. As a key member of our multi-disciplinary team, you will work closely with Consultant Psychiatrists, Psychologists, and other healthcare professionals to provide person-centred targeted support to patients with severe and enduring mental illness.

The successful candidate will have recent experience working in a mental health environment and be able to apply the philosophies of engagement and recovery in their work with service users. You will be responsible for completing regular clinics, depot, Olanzapine, and physical health assessments, as well as providing comprehensive care coordination for a caseload of patients.

We are looking for a motivated and committed individual with excellent communication and organisational skills, who is able to prioritise and work effectively in a fast-paced environment. Main Duties

  • Provide person-centred targeted support to patients with severe and enduring mental illness
  • Complete regular clinics, depot, Olanzapine, and physical health assessments
  • Provide comprehensive care coordination for a caseload of patients
  • Work closely with Consultant Psychiatrists, Psychologists, and other healthcare professionals to provide a holistic approach to patient care

Requirements

  • Recent experience working in a mental health environment

  • Excellent communication and organisational skills
  • Ability to prioritise and work effectively in a fast-paced environment

Working Hours

This post is for 37.5 hours per week, working Monday to Friday, 09:00hrs to 17:00hrs.
